If you are in-network with Bento:
(Bento partners with the DenteMax and Connection Dental networks)

1.  Create a login or sign into dentists.bento.net
2. Search for the member and confirm if they are Active
3. Download a copy of their dental plan summary
4. Create an appointment or estimate to confirm plan eligibility or patient pay total for any service.
